New Client Intake & Release Forms 

Covid-19 Clarifications and Declaration for IN-HOME services: Due to the 2020 Covid-19 pandemic faced by the U.S. which has been accompanied by strict shelter-in-place orders by local counties, GoldiLacts is taking extra precaution to ensure the health and safety of our support specialists as well as the safety of each family with whom they come into contact. We are conducting in-home services for families who have been discharged from the hospital with confirmed negative Covid testing results as well as those who have been practicing social distancing rules throughout the stay-at-home orders only. We will be practicing in-depth sanitization practices including frequent hand washing, mask-wearing when handling baby, social distancing when not having physical contact with mother and/or baby, and frequent sanitization of each product and piece of equipment used throughout the support session. Sessions will be rescheduled if your GoldiLacts support specialist is experiencing a temperature of 100.3 degrees or higher or any other symptoms of illness. Parents are to also inform GoldiLacts if they or anyone else in the home is experiencing a temperature of 100.3 degrees or higher and if so, the session will be rescheduled. If paid services cannot be fulfilled within two weeks due to GoldiLacts’ inability to provide services in good faith, you will be provided a full refund. By signing this release, you declare that we have been invited to provide these services in person, in addition to your understanding that should anyone in your family be effected directly by Covid-19 following the administration of our services. GoldiLacts is not to be held liable physically or financially. No refunds for services will be provided due to the possible onset of Covid-19 following the administration of our services. 

GoldiLacts does not bill insurance directly; your payment will be made to your lactation consultant. GoldiLacts does not guarantee insurance reimbursement. 

Handling forms and processing payments before our arrival allows us to have more hands-on, focused time with the breastfeeding parent and your newborn(s)! Reviewing our intake process allows you to make sure we are the right fit for your family, and also allows us to become familiar with you before we arrive. This helps all of us ease into the consultation must faster! Our goal is to make you as comfortable as possible in your own environment and leave you feeling much more confident in your own space with your baby.

Once you have clicked the “All Set!” button at the bottom of your intake forms, your information will have been received electronically. Payments are submitted via Venmo or PayPal. You will be taken directly to our confirmation page once the forms are complete and you will hear back from a lactation consultant within 24 hours! We look forward to working with you!

Payments for all services, workshops, and lactation support packages must be made in full at the time of scheduling to confirm your date/time of services. Sorry, no refunds.
